Laboratory Supply Terms and Conditions
AORTA AUSTRALIA PTY LTD
Overview
AORTA Australia Pty Ltd is the supplier of dental laboratory appliances for dental practices within Australia. The AORTA lab team to produce quality appliances for the dental practice patients using set policies and process workflows for producing quality items to its clients who are in direct contact with the end patient.
To produce such quality appliances AORTA has created its workflows following a strict guideline from researching the ADIA Laboratories Regulation Standards and TGA provisions.
Under Regulation 10.3 of the Therapeutic Goods (Medical Devices) Regulations 2002
- The Australia manufacturer of a custom-made medical devices from Australian supplied products.
AORTA follows this category with the supply of custom-made appliances in Melbourne, Australia.

- Introduction
The parties agree and acknowledge that the products and services outlined in these terms and conditions will be provided by AORTA Australia Pty Ltd.
- Lab Products & Services
AORTA will provide:
- Products and services to the Client in respect of Aesthetic, Orthodontic and Restorative lab services.
All products and services set out in the document provided with this document entitled “AORTA Australia Dental Lab Price List”.
- Supply Agreement
AORTA will provide:
(a) the Client products as ordered to the requested time frame allowing for the order to be received on a business day, by 12.00pm;
(b) pending the information provided by the Client is complete;
(c) allowing for order processing procedures;
(d) the Client to approve the order if required; and
(e) allowing for manufacturing time.
- Fees
The fees are set out in the document provided with this document entitled “AORTA Australia Dental Lab Price List”.
All fees quoted as exclusive of GST.
- Payment Terms
AORTA Australia will invoice for the services works and the Dentist will pay the invoice within 14 days of receiving the invoice.
- Warranty
AORTA warrants that:
(f) it has the power and is entitled to grant to the Client warranty for the product supplied for any manufacturing fault only.
(g) The Client is required to return the product including a warranty claim form for investigation of the claimed fault within 7 days of reporting.
(h) The Client will then be advised of the investigation outcome and communicated about the product replacement terms.
(i) Should the Client require an immediate replacement for the patient, this will be supplied and charges in holding until the faulty product is returned and deemed faulty by AORTA.
(j) Should the Client fail to return the faulty item the Client may receive an invoice due and payable for the replacement product.
- Documentation
Upon request AORTA can supply:
- Warranty Claim Form
- Product Care Instructions
- Product material safety data sheets

- Mentorship notice
AORTA Australia and its personnel are not familiar with important specifics of the matters or cases discussed during the mentorship services provided to the Practice. Any information conveyed by AORTA Australia or its personnel is not to be relied upon as clinical health care or supervisory advice in any way. The recipient is solely responsible for any use of the information communicated by AORTA Australia as an independent practitioner.
- Privacy
AORTA Australia while providing services, collects personal information about patients of the client. The data storage of any private information is secure on Australian server databases and not shared with any 3rd party organisations.
All data is stored in digital form and not removed from its secure location other than internal workings on the original file.
In the unlikely chance that a data breach is experienced, all clients will be notified, and an investigation will be held for the cause and immediate resolution.

- Consent
The Client places orders for products and services on behalf of the patient as their treating provider. The Client placing the order is responsible for any consent required by the patient for sharing their information to AORTA.
